What Is an All-On-4 Implant?

A dental implant is essentially a small (typically titanium) screw that is embedded into your jawbone in the gap left by a missing tooth. After it is set in position, Dr. Raptou attaches a false tooth on top of the screw. An All-On-4 implant is this same concept multiplied by four.

How Is an All-On-4 Implant Applied?

Rather than placing an implant for each missing tooth, we simply utilize four on top and four on the bottom as your teeth’s primary anchors. At that point, Dr. Raptou can connect a denture in those spots.

When Should I Not Get an All-On-4 Implant?

There are a wide variety of factors to consider when deciding whether an All-On-4 dental implant is right for you, and we can’t narrow them all down here. However, we will summarize four important variables that may lead you to re-consider getting the procedure:

•  Speech issues- This is often a major concern for patients, as what may at first seem like a minor speech alteration can become stressful later. Because the All-On-4 bridge requires both vertical & horizontal bulk for its strength, most bridges need 15 mm or more of height. Consequently, we may have to replace more than the volume of your missing teeth to reach the correct measurements. The procedure also often requires replacing bone and soft tissue volume, even if it’s still healthy. The portion of the bridge adjacent to the soft tissue can make it bulkier than the original bone and tissue volume it replaced. When your tongue makes contact with the hard palate lingual and central incisors or the horizontal bulk in the back, this can affect the way you make sounds (Ex- “D”, “T”, “S”, “N”) and slur your speech.
•  Difficulty adapting to bridge bulk- Some patients find it challenging getting used to the different feeling of an All-On-4 bridge, as they are used to the transition from their soft tissues to their teeth. When you feel the transition from those real tissues to the fake tissue portion of the bridge prior to feeling their teeth, this can lead to a psychological problem. Unfortunately, many patients only figure this out after surgery is over. At this point, the procedure cannot be reversed, and nothing can replace that feeling of the natural transition from tissue to teeth.
•  Proprioception- This is often overlooked but can be a big factor. Periodontal mechanoreceptors (PMRs) present in the periodontal ligament are what make teeth sensitive to certain physical sensations, but an implant lacks this ligament. It takes about ten times more force to register the same proprioception (perception or awareness of position & movement in the body) as a real tooth. A dental implant’s proprioception is similar to that of a tooth under local anesthesia, meaning you may have a harder time recognizing responses to physical contact in your mouth & jaw. Ex- You may unintentionally bite down too hard when eating from lack of feedback.
•  Parafunctional habits- These include bruxism, clenching your teeth, and irregular chewing, all of which may be impacted by the All-On-4. As said, implants will not recognize physical force as easily as a real tooth. Once the surgery is completed, a patient who already clenches or grinds their teeth may do so harder than before without realizing it.
